You may report suspected fraud through the Arizona Department of Housing’s Fraud Alert form below. If you wish to
remain anonymous, please do not provide any identifying information within the form, as any information provided may
be considered a public record subject to public disclosure in accordance with State Public Records laws. If you are
reporting potential fraud at a specific address, please provide the full address, including the name of rental properties (if
known), and the city in which they are located. The Arizona Department of Housing will investigate any matters over
which it has jurisdiction, and for those matters that it does not, it will attempt to provide the information to the proper
authority for review. You should not expect to receive any direct response to your submittal, but please be assured
that the Department will follow up in a timely and appropriate manner.
